Claim Process and Documents required in claim process

The insured must submit required document and evidence of accident to Muang Thai Insurance Public Company Limited or Dhipaya Insurance Public Company Limited Or the authorized person who represent both company within 30 days period or as soon as possible after the accident occur, if the insured submit documents after 30 days, the insured must have appropriate reason to convince Muang Thai Insurance Public Company Limited or Dhipaya Insurance Public Company Limited.

  2. Prepare documents required for each case.
  3. Submit the documents to Muang Thai Insurance Public Company Limited Or Dhipaya Insurance Public Company Limited
  4. The company considers the case and the coverage.

Required Documents for Claim
Indemnification for Compensation from loss of life

The Beneficiary have to send the below evidences at his own expense to the Company within 30 days from the date of death or the commencement of the disability.
  1. Claims form
  2. Death certificate
  3. Perform postmortem report
  4. Police report
  5. Copy of identification card and census registration (stamp “death”) of the insured
  6. Copy of Identification card and census registration of beneficiary

Indemnification for compensation from dismemberment, loss of sight or total permanent disability

  1. Claims form
  2. Doctor's certificate

Indemnification for Medical expense

  1. Diagnose and treatment report
  2. Original receipt for treatment expenses

Indemnification for Trip cancellation

  1. Doctor certificate, death certificate of
  2. 1.1 Insured
    1.2 Parents of Insured
    1.3 Husband/Wife of insured
    1.4 Parents of Husband/Wife of insured
    1.5 Children of insured
    Or other cancellation travel report from the transporter
  3. Original receipt for prepaid travel confirmation or prepaid hotel reservation, penalty invoice for additional charge which the insured must pay.

Indemnification for Hijacking

  1. Air ticket evidencing that the insured has been the passenger on the hijacked flight.
  2. Document assure for hijacking (If any)

Indemnification for Travel Delay

  1. Transporter's Delay report to explaining the delay reason.

Indemnification for loss/damage of baggage or personal effect

  1. Claim form
  2. Contact Nok Air‘s officer to take picture of damages of baggages or personal belongings.
  3. Loss confirmation document issued by carrier, location manager or accommodation owner which the insured stayed with and loss baggage incurred
  4. Refund document from carrier, transporter or location manager, accommodation owner
  5. Local police's event recorded which the insured reported

Indemnification for Third party liability

Third party injury​

  1. Doctor Certificate
  2. Medical expense receipt
  3. Copy of identification card of injured person.


Third party property damage​
  1. Repair receipt or other receipt for new buying
  2. Picture of the damage property (If any)

Remark: The company can request for other relevant documents for indemnification process. (The company reserves the right to inform the claimant/insured person later)
